Primary care Network

Primary Care Networks (PCNs) are groups of practices that work together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas.

NHS England’s Long-Term Plan outlined the ambition for Integrated Care Systems. The BMA GP (England) committee and NHS England agreed, through national contract negotiations, to develop and roll out of Primary Care Networks (PCNs).

There are nine PCNs in Bolton.  Deane Medical Centre is part of the Bolton Central Primary Care Network.  Bolton Central Primary Care Network is comprised of the following 13 practices

Deane Medical Centre, Beehive Surgery, Lever Chambers 1, Bradford Street Surgery, Great Lever HC , Olive Family Practice ,Pikes Lane 2 ,Orient House MC ,Pikes Lane 3 , Deane Clinic ,3D Medical Centre , Bolton General Practice ,Bolton Medical Centre .  As a Network we work collaboratively to improve patient access and extend the range of services available to you, and by helping to integrate primary care with wider health and community services.
